Multiple physiologic effects of fibroblast growth factor 21 <t>(FGF21).</t> Circulating FGF21 is a hepatokine secreted into plasma (central portion of figure) that interacts on target tissues through a receptor complex (panel b) comprised of fibroblast growth factor receptor family member 1 (FGFR1) and beta-klotho (KLB). The principal target tissues associated with metabolic effects are the central nervous system (CNS), liver and adipose tissue (panel a). Numerous studies in model systems, in vivo and from human genetics have supported the individual effects and pathways shown in the boxes for target tissues. Although the individual effects are complex and in some cases counteracting, the aggregated effects of FGF21 are largely beneficial encompassing such metabolic variables as reduced very-low density lipoprotein (VLDL) secretion and resulting triglyceride (TG) levels, decreased hepatic TG content increased peripheral lipolysis and free fatty acid (FFA) uptake, in addition to several beneficial effects mediated through probable CNS-liver and CNS-adipose axes.
